Wider Determinants Health (WDH)

Wider determinants of health are a range of social, environmental and economic factors which impact directly on the health (physical and mental) of the population. We have been working in partnership with the Black Country Healthcare NHS Foundation Trust (BCHFT) to identify how best to address some of the wider determinants of health, and how they impact on the wellbeing of our local residents. We are pleased to be delivering services which address some of these wider determinants of health and that help to support people who are in need.

At JST, our Wider Determinants of Health offer is to support to address health inequalities for people across Sandwell and Dudley. We will meet with you in the local community, review your current circumstances and health information, and explore what support is available in the local area. The support will be to help you increase your well-being, to take advantage of opportunities and resources available, focus on self-betterment and enhance life chances.  The most suitable and beneficial support may be with another JST project such as Platform or Workwell, or it could be with one of our partners on the project.

We will discuss all options with you and offer information, advice and guidance accordingly. Organisations contributing to the WDH project in Sandwell include JST, European Welfare Association, Sandwell African Caribbean Mental Health Foundation, Kaleidoscope Plus Group, Sandwell Youth and Community Centre, Beat it Percussion, St Albans Community Centre, Life in Community, Breakthru CIC, The Community Connect Foundation, Age UK Sandwell, SWEDA, Brushstrokes, YMCA Black Country Group, Ideal for All, Ilyes Community Centre, and Bangladeshi Women's Association.

Organisations contributing to the WDH project in Dudley include JST, Beacon Centre for the Blind (lead), CHADD, Without a Trace, The What? Centre, Advance Beyond, Aspire4u, Headway, Lighthouse Counselling, Phase Trust, The AOC, and Top Church Training. We work closely and competently with these partner agencies to assist service users to reach their goals and improve their health and lives. In addition, we work closely with other organisations in the Sandwell and Dudley boroughs and can signpost and refer based on the presenting needs. This can lead to making positive interventions, health-related or otherwise.

Advocate

We are able to speak up for individuals and act on their behalf and them in a range of complex situations such as benefit assessments, appeals, dealing with debt agencies, social services & housing.

Mentor

We provide support over a period of time to help and guide people through difficult situations, to overcome issues and barriers and change their lives for the better.

Support

We provide individual support for people facing a range of issues. Working 1-2-1 we work with people to identify and address their main barriers, develop coping skills, improve confidence and well being

Empower

Whilst working with people to address their issues and barriers we actively support them to become stronger, more confident and gain control over their own lives.
